About Dr Hornbake
Career Summary
Following completion of residency training I entered practice in a federally designated “Health Workforce Shortage” area of North Carolina. While in North Carolina I served in a number of regional and national leadership positions. Because of this experience I was recruited to lead a hospital system in Rochester NY. Subsequently I was asked to provide medical leadership in increasingly complex organizations. In 2002 I returned to Connecticut and to full time private practice.
In 2010 Dr Michael Baldwin, Dr Alan Spagnola and I formed Essex Internal Medicine, a group medical practice dedicated to excellence in medical care and patient service through the application of advanced information technology. In 2011 Essex Internal Medicine was recognized by the Medicare program as one of the first practices in Connecticut to achieve the lofty federal goals of advanced medical information management. At the same time the group was recognized by the National Committee for Quality Assurance (NCQA) as achieving the highest level of care using the Patient Centered Medical Home model for primary care services.
