Requesting an Appointment
You can use email to request an appointment. Please do not use email to address urgent problems since the mailbox is checked only periodically. Be sure to include your telephone number and full name in your email.
Prescription refill requests should be routed through your pharmacy. This is because we use an electronic prescribing system designed to reduce errors, improve record keeping and improve patient safety.
