PerspectiveIn Europe it is still common for physicians to visit patients in their homes. This style of practice has been largely lost in this country. There are a number of reasons why home visits should be preserved and expanded. Several large studies have shown substantial benefit from home visits. These benefits include a reduction in the risk of hospitalization, reduction in rates of adverse effects of medication, and reduction in mortality. In response the Medicare program has recognized this value and has enhanced benefit coverage for home visits. Home visits for whom?Seniors who find travel difficult benefit from home visits. Sometimes these visits are necessary only for a short time such as after discharge from a rehabilitation center or hospital. Other conditions make home visits necessary for a longer period. Many patients receiving long term home visits are also seen by the Visiting Nurses Association or other home care agency. However Medicare patients need not meet the federal definition of "homebound status" to be eligible for home visits by a physician. This status is required for home care agencies to bill Medicare. Home visits are usually scheduled well in advance. They do not work well for urgent or emergency problems. They are valuable in assuring that people with chronic treatable conditions are not lost to follow up. Younger patients with chronic conditions may also benefit from home visits. Health plans vary in their willingness to cover these visits however. Home visits are available in the following communities:
