Practice Data on Quality and Efficiency of Care are now available.

While the practice uses a patient registry system to track and manage chronic conditions, only data reported by external reviews of performance are posted online.

From United HealthCare based upon 2008 data.  View a press release related to this data.  To view Quality Data click here. The report indicates a near perfect quality score on a wide range of evidence based practice guidelines.

To view Efficiency of Care Data click here. The report is  based on 125 separate episodes of care.  The study analyzed the total cost of each episode of care including visit costs, xrays, prescriptions, laboratory studies and referral to other physicians.  The data indicate that Dr Hornbake’s care saves patients, employers and plan sponsors 31% compared  to average care in the community.

From Anthem based on 2008 data.  Click here to view the report.  The report shows a perfect 100 point score based upon a composite of quality, efficiency and practice process measures.

From HealthNet based on 2008 data. Dr. Hornbake has been given the Award for Exemplary Performance by HealthNet and by its partner,  the Connecticut State Medical Society.  This award is based upon superior quality and efficiency measures.

From the Connecticut Helath Quality Cooperative based on the pooled 2007 data from Medicare, Aetna, Anthem, Connecticare, HealthNet and United HealthCare Dr. Hornbake scored better than the average Connecticut physician in all six categories of measurement.  To view this report click here.
